What is market access in the pharmaceutical industry?

Market access covers everything that allows a medicine to be reimbursed, priced and genuinely available to patients. Obtaining authorisation to market a product is not enough: it still has to be covered by the health system, at a price that makes commercialisation viable. That is exactly the territory of market access.

Market access or “accès au marché”: what are we talking about

Both expressions describe the same function. “Accès au marché” is the official French translation used in occupational reference frameworks, but almost every pharmaceutical company uses the English term, including in its org charts and job titles.

One useful clarification for candidates searching online: in international trade, “market access” also refers to access to export markets and customs issues. The two worlds have nothing in common, which explains the frequent confusion in search results.

Market access and regulatory affairs: two distinct functions

The confusion comes up regularly, including among experienced candidates. Regulatory affairs obtains the marketing authorisation, in other words the right to sell. Market access obtains reimbursement and pricing, in other words the actual ability to sell.

The two functions work in sequence and coordinate early on, but the skills are not the same: regulatory rests on documentary compliance, market access on health economics and institutional negotiation. Why the function has gained weight

The rise of market access in org charts is explained first of all by the price of innovative therapies. In oncology as in gene therapy, the levels reached make proof of value unavoidable. Payers now demand health economic evidence that the clinical dossier alone does not provide. And spending controls have tightened, shifting the challenge from market authorisation towards negotiation.

The role and reporting line of the market access director

The market access director defines and drives the market access strategy for the entire product portfolio, from development phases through to the maintenance of coverage. They represent the company before health authorities and arbitrate between the company’s economic objectives and payer expectations.

Reporting line

The position most often reports to general management, to medical affairs, or to a corporate market access function in international groups. The reporting line matters: a market access function attached to medical affairs will have a scientific centre of gravity, while a direct line to general management means a seat on the executive committee and real weight in portfolio decisions.

France, Europe or global scope

A France market access director manages relations with the French National Authority for Health and the Economic Committee for Health Products. A European or global scope means coordinating affiliates across heterogeneous healthcare systems, building international reference pricing strategies and managing launch sequencing country by country. The compensation gap between the two configurations is significant.

The responsibilities of the market access director cover building the access strategy, submitting assessment dossiers to health authorities, negotiating pricing, producing evidence of value and managing the pricing, health economics and public affairs teams.

Building the access strategy from the development stage

Access strategy is prepared years ahead of launch. The market access director contributes to clinical trial design to ensure that the comparators, endpoints and populations selected will be acceptable to assessors. A methodologically flawless study can still be judged insufficient by the French National Authority for Health if the chosen comparator is not the one used in French practice.

Managing the transparency dossier

The dossier submitted to the Transparency Committee of the French National Authority for Health determines two decisive ratings: the actual clinical benefit, which sets the reimbursement rate, and the improvement in actual clinical benefit, which sets the room for negotiation on price. For products with a high budget impact, a health economic assessment conducted by the dedicated committee is added to this.

An improvement rating of level V, meaning no demonstrated progress, closes almost every pricing door. Most of the market access director’s work consists of avoiding that outcome.

Negotiating the price with the CEPS

The negotiation with the Economic Committee for Health Products covers the list price, but also rebates, volume clauses and risk sharing mechanisms. These agreements are confidential, which explains why the published price of a medicine rarely reflects its real price.

This phase calls for institutional negotiation skills that cannot be learned in a training course. It is the area where candidates are hardest to substitute.

Building the value proposition and using real world data

The market access director assembles the product’s value argument: clinical benefit, organisational impact, savings generated, response to an unmet need. Real world data plays a growing part in this demonstration, particularly for reassessments and for products that have benefited from early access.

Leading the teams

Depending on the size of the company, the director leads teams in pricing, health economics and outcomes research, public affairs and payer relations. In leaner structures, particularly in biotech, they handle part of these responsibilities personally, which completely changes the profile being sought.

Anticipating current reforms

Several recent developments weigh directly on the day-to-day work of market access teams in France.

The European regulation on health technology assessment, applicable since January 2025, introduces a joint clinical assessment at European level. It covers oncology medicines and advanced therapy medicinal products first, with progressive extension planned to orphan medicines and then to all medicines. In practice, part of the clinical analysis now takes place upstream and jointly, while the appraisal of value and price setting remain national. This does not reduce the workload of French teams, it shifts it and moves it earlier in the calendar.

The reform of derogatory access, which came into force in 2021, replaced temporary authorisations for use with early access and compassionate access schemes. It changed the sequence of availability and created a new challenge: data collected during the early access period then feeds the standard dossier.

Finally, the mechanisms regulating pharmaceutical spending, including the safeguard clause, have become a strategic parameter. They are not solely a matter for the finance function: they influence launch decisions and negotiating positions.

At equivalent level, geographic scope remains the most decisive factor. Next come portfolio size and the presence of high value products or first-time listings, which expose the postholder more. The variable component is generally significant on these roles, and biotechs sometimes offset a more modest base salary with equity incentive schemes.
